Planning Tools

These tools have been designed to provide a framework for discussion that promotes partnerships to benefit young children with disabilities & their families. They are intended to assist local cross-sector early childhood leadership efforts to support high-quality inclusion across programs and increase the access & meaningful participation of young children with disabilities. School district teams can use the Preschool Inclusion Self-Assessment Tool (and the accompanying Facilitator’s Guide) to reflect on past experiences working collaboratively to provide services to young children and families. Leadership teams can use the PATH Planning Tool to fully develop their team and create a vision, goals, and plan for preschool inclusion in their community.

 

Veteran/Established teams that have worked with SCPI and have already developed a PATH plan are encouraged to revisit their plan & Self-Assessment annually and use the Revised PATH process and Abbreviated PATH Plan to capture progress and updates.
